Sam is actually a really awesome, capable hunter. He's desperate to get his brother and the angel out of Purgatory, but he keeps running across Leviathans that need killing, innocent bystanders that need rescuing, and having to stop whatever he's really trying to do in order to take care of that instead. So he keeps being awesome and capable and the more he does, the guiltier he feels, as it seems like he's getting farther and farther away from his brother. As if he's choosing the world over his brother. Even though Dean would tell him to rescue the girl and kill the monster...he just feels really terrible about it.
