Cas and Dean escape Purgatory somehow, not too much worse for wear. Not a terribly long time has passed, months, maybe a year, but time passed the same on both sides. Dean's immediatly frantic trying to find Sam because he knows what happens when Sam's on his own (killed, demon blood addicted, vengeful, soulless, etc...)
Dean dreads finding that his brother's gone off the wagon again or that he's gone on some sort of revenge rampage (cause Dean knows through Cas that Sam's powers are still there, just dormant). But what he finds is so much worse. A broken little brother, more destroyed than even Lucifer had managed. Thin, pale, barely functioning. With no one left and nothing left worth getting revenge on, Sam had nothing to fight for and just gave up.
Dean (and Cas) have to try and fix him.
Also, Cas is mostly fixed. Angel crazy seems to equal something that seems more like enlightenment anyway.
